have you tried Doc Delamarter, i had ADR surgery in the trials years ago.
i beleive he takes all kinds of insurance, if anyone can get it approved his office can was my experience

i had a lamo way back in 1979, however the surgery never relieved anything, in fact i felt worse after that surgery. my surgeon said i would be in for many more surgeries and most likely fusion....
